On or about September 9, 2024, employees used an extension ladder on top of a scaffold for accessing the roof, exposing the employees to fall hazards of approximately 30 feet.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest system: 240 Main St., Northampton, MA. On or about September 9, 2024, an employee was observed working on a commercial structure engaged in roofing activities with unprotected sides and edges greater than 30 feet above the ground below and was not protected by either guard rails, safety net systems or personal fall arrest systems. Larochelle Construction, Inc was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1490160, citation number 1, item number 1, and was affirmed as a final order on 02/19/2021, with respect to a workplace located on Leary Dr., Holyoke, MA.
