Employees were exposed to entering this space without proper protection and being struck by moving parts. Employees were exposed to death or permanent disability. The space contained spinning and rotating paddles by a motor could seriously injure or kill unauthorized entrants that had not followed the PRCS entry safety pre-cautions Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.146(k)(1)(v): Provide the rescue team or service selected with access to all permit spaces from which rescue may be necessary so that the rescue service can develop appropriate rescue plans and practice rescue operations. The employer did not have employees trained in rescue and did not have rescue equipment available to rescue an employee. The employers policy was to call 911 to rely on outside personnel and not to enter the confined space. The company did not have the fire department visit the facility and evaluate the site and pre-plan if a rescue would occur. In the event of a need for rescue injury severity would be increased due to delay in rescue. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

The guard shall be such that it does not offer an accident hazard in itself.
