The employer does not ensure that employees are protected from fall hazards while accessing the billet caster run out table to clean/remove slag.  The violation most recently occurred on December 12, 2014, when an employee working to remove a build-up of slag near Gega torch #6 fell approximately 12 feet to the ground below the table.    Republic Steel was previously cited for violations of this occupational safety and health standard or its equivalent standard 1910.23(c)(1), which was contained in:    OSHA inspection number 941464, citation number 1, item number 2 and was affirmed as a final order on June 19, 2014, with respect to a workplace located at 1807 E. 28th St. in Lorain, Ohio.    OSHA inspection number 942971, citation number 2, item number 4 and was affirmed as a final order on April 21, 2014, with respect to a workplace located at 2633 8th St. NE in Canton, Ohio.    To abate this violation, the employer must ensure that fall protection is in place for employees accessing the billet caster run out table.

The employer did not ensure that mechanical and flammable gas energy sources were isolated/controlled.    Republic Steel was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(d)(3), which was contained in OSHA inspection number 942545, citation number 2, item number 7 and was affirmed as a final order on April 14, 2014, with respect to a workplace located at 401 Rose Ave. SE in Massillon, Ohio.
