29 CFR 1926.454(c)(3): Inadequacies in an employee's work involving scaffolds indicates that the employee has not retained the requisite proficiency: a) 2303 Western Ave Guilderland NY - On or about December 12th, 2014 - Two employees were exposed to fall hazards while operating an aerial lift. A personal fall arrest system was not worn and/or a lanyard was not attached to the boom or basket when working from the aerial lift. The employees were trained in the use of aerial lifts yet the use of the aerial lift indicates that the employee did not retain the requisite proficiency.
