29 CFR 1910.179(n)(3)(ii)(c): The hook was not brought over the load in such a manner as to prevent swinging. a. Republic Steel located in Lorain, Ohio: On or about December 29, 2015, employees using the 75 ton overhead crane located in the tundish build-up area of the billet caster building were not bringing the crane hook directly over the tundish mechanisms, thereby causing the approximately 1000 pound mechanisms to swing once they were removed from the tundish.

29 CFR 1910.184(e)(3)(i): In addition to the inspection required by paragraph (d) of this section, a thorough periodic inspection of alloy steel chain slings in use were not made on a regular basis, to be determined on the basis of (A) frequency of sling use; (B) severity of service conditions; (C) nature of lifts being made; and (D) experience gained on the service life of slings used in similar circumstances. Such inspections were at intervals greater than once every 12 months. a. Republic Steel located in Lorain, Ohio: On or about December 29, 2015, the employer did not ensure a chain sling used in the tundish build-up area of the billet caster building, was inspected annually. The chain sling is used by employees to remove approximately 1000 pound mechanisms from the tundish.
