Republic Steel located in Lorain, Ohio:  On or about December 29, 2015, employees using the 75 ton overhead crane located in the tundish build-up area of the billet caster building were not bringing the crane hook directly over the tundish mechanisms, thereby causing the approximately 1000 pound mechanisms to swing once they were removed from the tundish.

Such inspections were at intervals greater than once every 12 months.    a. Republic Steel located in Lorain, Ohio:  On or about December 29, 2015, the employer did not ensure a chain sling used in the tundish build-up area of the billet caster building, was inspected annually.  The chain sling is used by employees to remove approximately 1000 pound mechanisms from the tundish.
