29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms shall be kept clean and orderly and in a sanitary condition. a. Throughout the facility - On or about December 30, 2014, employees were required to work in the shipping and receiving lanes on a daily basis. The floors of the lanes were covered with shipping containers, packing material, broken pallets, wood slats with nails protruding, and other debris. Employees were thereby exposed to hazards associated with impalement, slip / trip / fall, and struck-by.

29 CFR 1910.178(n)(1): All traffic regulations shall be observed, including authorized plant speed limits. A safe distance shall be maintained approximately three truck lengths from the truck ahead, and the truck shall be kept under control at all times. a. On or about December 31, 2014, employees were required to operate powered industrial vehicles (PIVs) on a daily basis throughout the facility. The employees operating the PIVs did not maintain a safe distance, approximately three truck lengths from the truck ahead and did not ensure that the trucks were kept under control at all times. The employer failed to ensure that all traffic regulations, as required by 1910.178, were observed. Employees were thereby exposed to the hazards associated with PIVs being operated in an unsafe manner.
