29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition:    a) On and at times prior to December 30, 2014 the dock floor was not kept in a clean and sanitized condition, especially at the southwest corner of the dock with pigeon dropping collected near the exit door.    b) On or about December 20, 2014 broken pallets and pieces of wood were present on floor at the west end of the building.

29 CFR 1910.303(h)(2)(v)(B): 1910.303(h)(2)(v)(B): Suitable guards were not provided for equipment exposed to physical damage from vehicular traffic:     a) On or about December 30, 2014 the electrical transformer located in front of the exit door on the north side of the building was not protected from potential vehicular damage as the trailer was backed into the dock door within 3 feet of the unit.

29 CFR 1910.178(q)(1): Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel:    a) On and at times prior to December 30, 2014 the P1219 LP forklift was missing the cylinder tank set pin.      b) On and at times prior to December 30, 2014 the P1861 LP forklift was missing the cylinder tank set pin.      c) On and at times prior to December 30, 2014 the P1862 LP forklift did not have a secured set pin in place.      d) On and at times prior to December 30, 2014 the P1867 LP forklift had a damaged cylinder tank set pin.       e) On and at times prior to December 30, 2014 the P825 LP forklift had a damaged cylinder tank set pin.     The UPS Freight was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.178(q)(1) which was contained in OSHA inspection number 980950, citation number 02 , item number 001 and was affirmed as a final order on December 11, 2014, with respect to a workplace located at  1370 Goodyear, El Paso, TX 79936.    The UPS Freight was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.178(q)(1), which was contained in OSHA inspection number_315298026, citation number 01, item number 001 and was affirmed as a final order on June 29, 2011, with respect to a workplace located at 31 Sterling Road, North Billerica, MA 01862.

29 CFR 1910.22(b)(1): Where mechanical handling equipment was used, sufficient safe clearances were not allowed for aisles, at loading docks, through doorways and wherever turns or passage had to be made:    a) On or about December 30, 2014 metal railing from existing overhead dock doors were protruding into the forklift path creating struck-by and unstable load hazards at dock doors such as but not limited to 5, 37, and 38.     b) On or about December 30, 2014 uneven concrete pavement in the yard in front of door 37 created a tripping hazard for employees exited their truck cab.    The UPS Freight was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.22(b)(1), which was contained in OSHA inspection number 315886697, citation number 01, item number 001 and was affirmed as a final order on February 17, 20112, with respect to a workplace located at 1601 Toronita St. York, PA 17402-9922.    The UPS Freight was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.22(b)(1), which was contained in OSHA inspection number 315496455, citation number 02, item number 001 and was affirmed as a final order on July 15, 2011, with respect to a workplace located at3250 East Kemper Rd. Sharonville, OH 45241.
