OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:      A.)  Allegheny Surplus Outlet, Inc., 10921 Murray Road, Meadville PA 16335,  office / break room area, on or about December 30, 2014, and times prior thereto:  Employees were exposed to fire hazards and fall hazards from the lack of emergency illumination.  The passageway / exit route running between the office area / break room area was not equipped with devices to provide adequate lighting during a power outage.  The exit route was not configured so that windows could provide supplemental illumination. On December 30, 2014, a power outage occurred which left the area in total darkness.      B.)  Allegheny Surplus Outlet, Inc., 10921 Murray Road, Meadville PA 16335, warehouse area, on or about December 30, 2014, and times prior thereto:  Employees were exposed to fire hazards and fall hazards from the lack of emergency illumination. OSH ACT of 1970 Section (5)(a)(1):     The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to improperly maintained exterior walking/working surfaces:    a) Allegheny Surplus Outlet, Inc., 10921 Murray Road, Meadville, PA 16335, front parking area near main entrance, on or about December 30, 2014, and times prior thereto:  Several pot holes in the asphalt/gravel parking lot accumulated water and formed ice patches.  The pot holes created a slip/ fall hazard for employees.    ABATEMENT NOTE:  Among other means, acceptable and feasible methods to correct the conditions include:  (1) Utilize recognized construction methods and techniques to repair the parking lot surface to eliminate the potholes and the slip, trip and fall hazards.  (2) Institute a regular inspection program for exterior walking/working surfaces in the parking lot area.  The inspection program should be designed to identify slip, trip and fall hazards associated with potholes.
