Allegheny Surplus Outlet, Inc., 10921 Murray Rd, Meadville, Pa, 16335-Shipping area (south wall), on or about December 30, 2014, and times prior thereto: Areas of the shipping department were not maintained in a sanitary condition in that, three Sterilite brand containers contained accumulations of stagnant rainwater from a leaking roof. The containers displayed signs of potential fungal, and/or bacterial growth. No cleaning schedule was in place for the containers.

Allegheny Surplus Outlet, Inc., 10921 Murray Rd, Meadville, Pa, 16335-Warehouse area, on or about December 30, 2014, and times prior thereto: Employees operated forklifts including, but not limited to, a Yale brand powered industrial truck (Model # GLP050R, Serial # A875B23601A) in order to move merchandise around the warehouse area of the facility. The Yale #GLP050R industrial truck was not being examined daily or after each work shift. b.) Allegheny Surplus Outlet, Inc., 10921 Murray Rd, Meadville, Pa, 16335-Warehouse area, on or about December 30, 2014, and times prior thereto: Employees operated forklifts including, but not limited to, a Caterpillar brand powered industrial truck (Model # GC25, Serial # 4EM09753) in order to move merchandise around the warehouse area of the facility. The Caterpillar # GC25 industrial truck was not being examined daily or after each work shift. The Allegheny Surplus Outlet, Inc. was cited for a violation of this occupational safety and health standard or its equivalent standard 1910.178 (q) (7), which was contained in OSHA inspection number 311876262, citation number 3, item number 2, and was affirmed as a final order on October 18, 2010, with respect to a workplace located at 10921 Murray Rd, Meadville, Pa, 16335.
