29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  (a)  In the refrigeration section:  On January 6, 2015 and at times prior to; the employer exposed his employees to potentially hazardous energies in that employees were allowed to work on the refrigeration system which has multiple sources of energy such as electrical and pressurized anhydrous ammonia and water without those employees having a machine specific lock-out procedure to ensure those energy sources were controlled while the employees were performing maintenance or servicing on the system.   (b)  In the peeling room:  On January 6, 2015 and at times prior to; the employer exposed his employees to potentially hazardous energies in that employees were allowed to work on pressure systems that control the conveyors in that the employees were required to drain pneumatic pressure lines (120 psi) and lock out compressors prior to conducting maintenance without those employees having a machine specific lock-out procedure to ensure those energy sources were controlled while the employees were performing maintenance or servicing on the system.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  Deep Sea foods, Inc., Bayou La Batre:  On January 6, 2015 and at times prior to; the employer failed to ensure that machine specific lockout procedures were annually audited/evaluated to ensure procedures effectively control all applicable energy sources and to ensure that all employees follow established procedures.

29 CFR 1910.305(b)(2)(i): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose:  (a)  Outside on the Northwest side of the refrigeration system:  On January 6, 2015 and at times prior to; the employer exposed employees to electrical hazards in that employees were allowed to walk and work around an electrical junction box that did not have a cover installed, leaving wires carrying 110VAC exposed.   (b)  Inside the refrigeration compressor room:  On January 6, 2015 and at times prior to; the employer exposed employees to electrical hazards in that employees were allowed to walk and work around an electrical switch box that did not have a cover installed, leaving wires carrying 110VAC exposed.
