29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:  (a) Production area- An employee operating a radial arm saw was exposed to continuous noise at 80.1 % of the permissible daily exposure of 85 dBA (8-hour time weighted average) or an equivalent sound level of 88.4 dBA during the 409 minute sampling period on January 29, 2015. The exposure calculation included a zero increment for the 71 minutes not sampled.   (b) Production area- An employee operating a radial arm saw was exposed to continuous noise at 71.4 % of the permissible daily exposure of 85 dBA (8-hour time weighted average) or an equivalent sound level of 87.6 dBA during the 441 minute sampling period on January 29, 2015. The exposure calculation included a zero increment for the 39 minutes not sampled.   (c) Production area- An employee operating a radial arm saw was exposed to continuous noise at 97.9 % of the permissible daily exposure of 85 dBA (8-hour time weighted average) or an equivalent sound level of 89.9 dBA during the 401 minute sampling period on January 29, 2015. The exposure calculation included a zero increment for the 79 minutes not sampled.    (d) Production area- An employee operating a radial arm saw was exposed to continuous noise at 74.3 % of the permissible daily exposure of 85 dBA (8-hour time weighted average) or an equivalent sound level of 87.9 dBA during the 459 minute sampling period on January 29, 2015. The exposure calculation included a zero increment for the 21 minutes not sampled.   An effective hearing conservation program would include, initial baseline and annual audiograms, making hearing protectors available, and initial and annual noise training.

29 CFR 1910.1200(h)(3)(iv): The details of the hazard communication training did not include an explanation of the Globally Harmonized System (GHS) with respect to the labeling, pictograms, and safety data sheets, including the order of information and how employees could obtain and use the appropriate hazard information:  (a) Throughout the facility- The employer did not provide training to employees on the new hazard communication standard Globally Harmonized System (GHS) as a result, employees were not familiar with the new standard, safety data sheets, and pictograms.  Employees were exposed to hazardous chemicals in the workplace, such as but limited to, coolants (Kool Mist containing triethanolamine), isopropyl alcohol and lubricants.
