29 CFR 1910.212(a)(3)(iii): Special hand tools for placing and removing material(s) was not provided to permit easy handling of material without the operator placing a hand in the danger zone: a) Production Area: On or about June 5, 2019, and at times prior, the employer exposed employees to amputation hazards for employees operating a band saw, in that the employer did not provide a push stick/shop aid so that when the guard was removed, the operator's hand was placed at the point of operation for a Delta band saw (Model# 20, S/N# 86303930).

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) Production Area: On or about June 05, 2019, and at times prior, the employer exposed employees to amputation hazards for employees operating a band saw, in that the employer did not ensure that the Delta Band Saw (Model# 20, S/N# 86303930) point of operation guard was utilized when making parts that vary in size. Gardner Metal Systems, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(3)(ii) which was contained in OSHA inspection number 1267445, citation number 1, item number 3 and was affirmed as a final order on April 12, 2018, with respect to a workplace located at 4276 Cantrell Road NW, Acworth, Georgia 30101.
