29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent as specified in paragraph (e)(3) of this section):    (a) Slump Station #1: On or about January 6, 2015, and times prior there to, employee(s) were exposed to a fall hazard of greater than four feet from the ground below while standing on a working platform attached to a slump stand, which was not equipped with sufficient guardrails on the front side of the platform.    (b) Slump Station #2: On or about January 6, 2015, and times prior there to, employee(s) were exposed to a fall hazard of greater than four feet from the ground below while standing on a working platform attached to a slump stand, which was not equipped with sufficient guardrails on the front side of the platform.

29 CFR 1910.1200(f)(5)(i): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the identity of the hazardous chemical(s) contained therein:    Truck Bay: On or about January 6, 2015, employee(s) where exposed to chemical hazards from two 55-gallon chemical drums(metal), containing chemicals including but not limited to Hypoid Gear Oil SAE 85W-90/85W-140, which were not clearly marked with chemical contents or hazard specifications, and were accessed by company employees for concrete truck maintenance.

29 CFR 1904.32(a)(1): A review of the OSHA Log of Recordable Work-Related Injuries and Illnesses (OSHA Form 300 or equivalent) was not completed to ensure the Log was accurate and complete, and/or deficiencies were not identified and corrected at the end of each calendar year.  Ingram readymix, Inc. did not record the following workplace injury correctly on the OSHA Form 300:    The employer did not ensure that entries on the OSHA Form 300 are complete and accurate or corrections are made to any deficiencies identified.  This violation occurred on or about December 31, 2014 when Columns (G) thru (H) were not completed to identify the most serious outcome of an employee's hand injury on the 2014 OSHA Form 300 for the Ingram Readymix, Inc. facility located at 18679 N.W. Military Hwy., San Antonio, TX, 78257.
