Sources of ignition may include open flames, lightning, smoking, hot surfaces, frictional heat, sparks (static, electrical, and mechanical), spontaneous ignition, chemical and physical-chemical reactions, and radiant heat:    Outside at Diesel Storage area , On or about January 6, 2015, and at times prior thereto, employees were exposed to burn hazards when parking next to the diesel storage tank while smoking cigarettes in the work trucks.

29 CFR 1910.303(f)(2): Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident:    Break room office shop area, On or about January 6, 2015, and at times prior thereto, employees were exposed to a fire hazard from an 110volt/20 amp circuit that was not labeled at the over current protection device in the electrical panel while using 2 microwaves and a toaster to heat food during break times.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:    South side of Truck shed, Air compressor area, Ingersoll-Rand (Model: 2545 Serial: 02028795 220 Volt/3 phase/40 amp circuit): On or about January 6, 2015, and at times prior thereto, employees were exposed to electrical shock hazards from an open electrical knockout measuring approximately 1 inch on an 4 inch by 4 inch metal junction box, while servicing an air compressor.
