At the jobsite - On or about January 7, 2015, an employee was using a Hitachi pneumatic nail gun to install roof shingles without the use of eye protection, thus exposing the employee to eye injuries.

At the jobsite - On or about January 7, 2015, there was an employee on the roof installing roof shingles without any fall protection system in place, thus exposing the employee to a fall hazard of up to 19.4 feet to earthen ground.

Northeast side of the house- On or about January 7, 2015, the ladder section used to gain access to the roof did not extend 3 feet above the roof landing surface, thus exposing the employee to a fall hazard of 9 feet onto earthen ground.
