29 CFR 1910.27(c)(2): A clear width of at least 15 inches (0.4 m) was not provided each way from the centerline of ladder(s) in climbing space(s):    On or about September 30, 2014 and thereafter, a clear width was not provided of at least 15 inches from the centerline of fixed ladder(s) used to access the pit(s) beneath power presses such as, but not limited to the H&F TEF 2500 E (Transfer 2500 #1). Employees were exposed to fall hazards.      Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.27(d)(2): "Landing platforms." When ladders are used to ascend to heights exceeding 20 feet (except on chimneys), landing platforms shall be provided for each 30 feet of height or fraction thereof, except that, where no cage, well, or ladder safety device is provided, landing platforms shall be provided for each 20 feet of height or fraction thereof. Each ladder section shall be offset from adjacent sections. Where installation conditions (even for a short, unbroken length) require that adjacent sections be offset, landing platforms shall be provided at each offset.    On or about September 30, 2014 and thereafter landing platforms were not provided where installation conditions required fixed ladder(s) to be offset to provide access the pit(s) beneath power presses such as, but not limited to the H&F TEF 2500 E (Transfer 2500 #1). The work surface between ladders was uneven and created a hazard to employees transitioning between fixed ladders.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.27(d)(3): The employer did not ensure that the side rails of a through ladder extended 3 1/2 feet above parapets and landings, omitting the rungs from the extension and having not less than 18 nor more than 24 inches clearance between the rails:     On or about September 30, 2014 and thereafter the side rails of fixed ladder(s) did not have an extension of at least 3.5 feet to safely access the pit(s) beneath power presses such as, but not limited to the H&F TEF 2500 E (Transfer 2500 #1). Employees were exposed to a fall hazard from the footing(s) to the bottom of the pit(s).      Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
