29 CFR 1926.501(b)(1): Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall protection.    a) Right Side Front, Second Floor of Structure - Four workers were working on and around the edge taking measurements in preparation to lay the plywood subflooring, not protected from falling 16' to the ground and scrap materials below, on or about 01/13/2015.       For the purposes of this inspection, this employer was determined to be the controlling and correcting employer of the jobsite. The employer is being cited under OSHA's Multi-Employer Workplace Policy.     Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    a) Right Rear of Structure - A job site constructed wood ladder used to gain access from the outside ground to the first floor level had one rail extended approximately 6" past landing surface, exposing employees to a 20' fall hazard on or about 01/13/2015.     For the purposes of this inspection, this employer was determined to be the controlling and correcting employer of the jobsite. The employer is being cited under OSHA's Multi-Employer Workplace Policy.     Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. 29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step:    a) First Floor Front Right Side - Workers were using a six foot fiberglass step ladder to gain access to the second floor by standing on the top step and climbing through exposed floor/ceiling joist, on or about 01/13/2015.    For the purposes of this inspection, this employer was determined to be the controlling and correcting employer of the jobsite. The employer is being cited under OSHA's Multi-Employer Workplace Policy.     Abatement certification required within 10 days after abatement date.
