29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    a.) Damar Products, Inc. located at 516 Parks St. Sidney, OH; On or about January 14, 2015, the employer did not ensure a program consisting of energy control procedures, employee training, and periodic inspections was implemented. Employees were required to change blades and perform maintenance on the band saws and cut off saw and were exposed to hazards from unexpected start up of the equipment.

Damar Products, Inc. located at 516 Parks St. Sidney, OH; On or about January 14, 2015, the employer did not ensure the Rockwell International table saw was properly guarded. Employees are required to use the saw to cut notches in the wood pieces to allow for banding, and are exposed to amputation hazards.
