29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a) Third Floor, Stairwell A - On or about 1-15-15 - An employee was injured while he was working on a eight inch pressurized steam line being tested for leaks. The system was pressurized to 100 psi and was not locked and tagged out. The air pressure was not released from the system and verified prior to the employee removing a blank at the end of a section of the eight inch pipe.
