Job Site - On or about 3-18-15 - The employer did not instruct employees in the proper tools to be used to install bits on a Milwaukee coring machine or to inspect drill bits for surfaces that are not smooth which could create a hazard by snagging a loose article of clothing or a glove.

29 CFR 1926.300(a): Hand tool(s), power tool(s) or similar equipment were not maintained in a safe condition: a) 777 Main Street, Hartford, CT. Job Site - On or about 3-18-15 - The drill bit for the electric Milwaukee coring machine was not maintained in a safe condition. The top of the three-inch drill bit was not maintained with a smooth surface, as a pipe wrench used to tighten and loosen the bit generated teeth marks on the bit. The teeth marks were enough to snag the employee's glove and shirt wrapping his arm around the bit breaking the employee's arm in two places.
