29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:    a) Grinding area    One employee operating the grinding machine for processing defective materials was exposed to continuous noise at 69.6% of the permissible exposure limit (PEL) eight hour time-weighted average sound level of 90 dBA.  The action level is 85 dBA.  The equivalent sound level of the measured dose is  87.3 dBA.  Sampling was performed for 448 minutes on 2/25/2015, with the un-sampled portion of time considered to represent zero exposure for the exposure calculation.      Violation occurred on or about 2/25/2015.    Abatement Note:  The employer shall implement a comprehensive hearing conservation program whenever employee noise exposures equal or exceed the action level.  A hearing conservation program includes at a minimum the following items:        1.  Exposure monitoring      2.  Use of hearing protection      3.  Audiometric testing and evaluation      4.  Employee training      5.  Recordkeeping
