29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Production area The employer did not develop and utilize energy control procedures for use during maintenance and mold set up for plastic injection molding machines, thereby exposing employees to amputation hazards. On 1/23/2017, an employee performing maintenance on Nissei 289 ton plastic injection molding machine #6 (s/n S26E054) suffered amputation of 2 fingers when the mechanical safety bar engaged crushing employee's hand. Condition occurred on or about 1/23/2017
