Employees setting masonry block for interior and exterior walls, for the second and third floors of the project, were not protected from falls greater than 10 feet to the surface below by any means of guardrails, safety nets or personal fall protection. Employees framing the floor joists for the second and third floors were not protected from falls between the joists, to the surface below by any means of guardrails, safety nets or personal fall protection.

Employees were setting masonry block on the third floor of the project and were not protected from falling off the leading edge or through stairwell openings 10 feet to the surface below by any means of floor covers, guardrails, or personal fall protection systems. Employees framing floors on the second floor level were not protected from falling off the leading edge or through floor openings, by any means of floor covers, guardrails, or personal fall protection systems.
