29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    Location: 579 Van Buren Street, Brooklyn, NY 11222    On or About January 15, 2015    a). Employees setting masonry block for interior and exterior walls, for the second and third floors of the project, were not protected from falls greater than 10 feet to the surface below by any means of guardrails, safety nets or personal fall protection. Employees framing the floor joists for the second and third floors were not protected from falls between the joists, to the surface below by any means of guardrails, safety nets or personal fall protection.

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around each floor hole.    Location: 579 VanBuren Street, Brooklyn, NY 11222, East side of project.        A). Employees were setting masonry block on the third floor of the project and were not protected from falling off the leading edge or through stairwell openings 10 feet to the surface below by any means of floor covers, guardrails, or personal fall protection systems.  Employees framing floors on the second floor level were not protected from falling off the leading edge or through floor openings, by any means of floor covers, guardrails, or personal fall protection systems.

29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall or come against, was not guarded to eliminate the hazard of impalement:    LOCATION: 579 Van Buren Street, Brooklyn, NY West side ground floor    On or about January 15, 2015.    a). Employees working along the second and third floors of the project were exposed to impalement hazards because protruding rebar was not capped.
