OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees including burns, shocks, and electrocution: (a) On or about January 15, 2015, and at times prior, employees who worked in both dry and wet and damp locations were exposed to electrical hazards in that there were missing knockout plugs in receptacles, open junction boxes, strain on wiring components, missing switch covers, and spliced electrical wiring in the following locations throughout the plant: 1) In the package assembly room, strain on the electrical cord for lift truck model W40Z, 110V. 2) In the back wall of the metal detector room, wall receptacle missing knockout plug, 110V. 3) In the metal detector room, pendant receptacle missing knockout plug, 110V. 4) In the product cooler room next to the loading dock, open junction box, no cover, 110V. 5) In the product cooler managers office, open receptacle, no cover, 110V. 6) Outside of grow room #4, open receptacle, wet location, 480V. 7) Outside of grow room # 16, strain on cord at bottom of panel, wet location. 8) Outside of grow room #16, strain on cord for water pump #6 where the cord enters the motor, Dayton model 1D878, 240V, wet location. 9) Outside of grow room # 11, missing switch cover, wet location, 110V. 10) Outside of grow room #9, missing switch cover, wet location, 110V. 11) Outside casing grow room # 2, strain where cord enters the plug of humidifier pump, wet location, 110V. 12) Outside of spawn room # 3, missing switch cover, wet location, 110V. 13) In the tunnels area, spliced winch puller cable, cable connects to Appleton AR 20044 480V receptacle. 14) In the tunnels area east wall, open junction box, no cover, wet location, 110V. 15) In the tunnels area main power room, missing knockout plug on switch box, 110V. (b) On or about January 15, 2015, and at times prior, employees worked and operated equipment in the wharf and Ricking areas, where the structural stability of the supporting and connecting steel beams had not been verified after damage to the support structure, exposing employees to crush-by hazards. OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards: (a) On or about January 15, 2015, and at times prior, employees were exposed to fall hazards in the following locations: 1) In the maintenance shop mezzanine, deficient guardrails on the platform- top rail at 48", mid rail at 24", 30" wide gate, existing guardrails not strong enough to sustain a 200 Lb force. 2) The spawn-casing line platform showed structural deficiencies in that: (i) No guardrails on the right side of the access ladder. (ii) Platform steel support leg dislodged, tilted, corroded, and propped up to sustain platform with a weight of over 4,000 lbs. (iii) Access ladder missing the right footing, corroded to the point where it does not support the ladder.
