29 CFR 1910.178(q)(7): Powered industrial truck(s) which were used on a round-the-clock basis were not examined for defects after each shift. Industrial trucks were being placed in service after the examination showed condition(s) adversely affecting the safety of the vehicle: a. Even though forklift operators had been trained to conduct an informal inspection of forklifts, the company had established an inspection procedure, using a written, documented checklist, which was relied upon by maintenance to conduct the inspections. That procedure was not implemented for all of the forklifts in operation, including the Nissan and Toyota forklifts. In addition, the established inspection process was not completed before each shift where forklifts were operated on all three shifts. Inadequate inspection was indicated by operation of the Nissan forklift even though a propane tank support strap was missing for weeks, according to employees. Inadequate inspection was also indicated by elevated exhaust levels and a leaking propane fuel line. Peak levels of carbon monoxide were measured at 68 ppm in the warehouse on 1-20-15. Abatement documentation is required for this violation.

29 CFR 1910.178(q)(1): Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel: a. The Nissan forklift was operated with a missing propane tank support strap. b. The Nissan forklift was operated with a propane line component in disrepair allowing propane to escape. (In addition the exhaust from the Nissan forklift was emitting elevated levels of carbon monoxide, measured at 68 ppm on 1-20-15.) Abatement documentation is required for this violation.
