29 CFR 1910.22(a)(2): The floor of every workroom was not maintained in a clean and, so far as possible, a dry condition. On 8/6/2015 and 8/11/2015, Continental Casting, L.L.C. had not ensured the floors in the work areas were maintained in a clean and, so far as possible, a dry condition in the aluminum and zinc casting areas. There was hydraulic fluid or lubrication fluid leaking from multiple machines. Several ovens had piles of aluminum scrap in front of the doors as well as scrap scattered around the machines.

29 CFR 1910.132(d)(1)(i): Select, and have each affected employee use, the types of PPE that will protect the affected employee from the hazards identified in the hazard assessment. On 8/6/2015 and 8/11/2015, Continental Casting, L.L.C. had not ensured that employees used the types of PPE that would protect them from hazards identified and required by the hazard assessment and the Continental Casting Personal Protective Equipment Work Instruction. Pot tenders and die casters were not required to wear the appropriate personal protective equipment.
