The CAPU processes acetonitrile. Employees were at risk of exposure to acetonitrile.

29 CFR 1910.119(f)(1)(i): The employer did not develop and implement written operating procedures that provide clear instructions for safely conducting activities involved in each covered process. (a) INEOS USA LLC, 1900 Fort Amanda Road, Lima, Ohio: On or about January 13, 2015, the employer did not develop and implement a clear operating procedure for shutting down the reactor and Absorber Off Gas Incinerator (AOGI). While shutting down the process, the AOGI's safety features activated releasing Acrolein, Acrylonitrile, and Hydrogen Cyanide to the atmosphere when liquid entered the AOGI affecting the unit's burner operation. The operating procedure did not provide clear instructions for adjusting the material feed and air flow rates. Employees were at risk of serious injury from exposure to hazardous chemicals including but not limited to hydrogen cyanide and acrylonitrile.

29 CFR 1910.119(j)(5): The employer did not correct deficiencies in equipment that were outside acceptable limits (defined by the process safety information on paragraph (d) of this section) in a safe and timely manner when necessary means where taken to assure safe operation: (a) INEOS USA LLC, 1900 Fort Amanda Road, Lima, Ohio: On or about March 18, 2015, the employer did not correct deficiencies in the North Drum cyanide circulating pump. The pump leaked hydrogen cyanide to the work environment and was used as a backup to the primary pump. Employees were at risk of serious injury from exposure to hydrogen cyanide.

(a) INEOS USA LLC, 1900 Fort Amanda Road, Lima, Ohio: On or about March 18, 2015, a management of change was not implemented and documented for a change involving a leaking hydrogen cyanide pump on the North Drum. The pump remained in service and was switched to a backup function instead of the primary pump. Operators used a steam lance to dissipate released hydrogen cyanide.
