(b) On or about 1/21/15 - Shakeout line, an employee(s) working at the shakeout line had not received a current FIT test with the model or make of respirator that was required to be utilized. The employee was provided and utilized a Gateway N95 respirator. (c) On or about 2/6/15 - an employee(s) working at the shakeout line and required by the employer to utilize a Gateway N95 respirator had not received a current fit test.
