29 CFR 1926.403(i)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by cabinets or other forms of enclosures, nor by any of the means listed in subparagraphs (A) through (D) of this paragraph:       Inside the office building under construction, employees used an electrical outlet that was not equipped with a cover to power a vacuum and an electrical chisel, exposing employees to electrical hazards.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:      At the worksite, on the roof of the ready-make building, the employee on the roof was not protected from falls of approximately 16 feet, exposing employee to fall hazards.
