29 CFR  1926.501(b)(11):Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  On or about July 17, 2023 the employer IAM Construction LLC, at jobsite located at 420 Avon Belden Rd. Avon Lake OH 44012, did not ensure that employees are adequately protected from fall hazards while conducting construction roofing activities. Employees were exposed to falls of 25 feet or greater from the roof of the structure to the ground below. Employees were working atop a steep roof with a 12:12 pitch at the peak of the roof.

29 CFR  1926.503(b)(1):The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  On or about July 17, 2023 the employer IAM Construction LLC, at jobsite located at 420 Avon Belden Rd. Avon Lake OH 44012, did not develop or maintain a written record of certification for all employees who might be exposed to fall hazards.
