29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: At the location of 309 E. Douglas Ave in Wichita KS, an employee engaged in sheet metal work on a flat roof was exposed to fall hazards of approximately 8 feet 6 inches. The employee was exposed in that the fall protection system the employee was wearing was not anchored to an anchor point to keep him from falling off the edge he was working on.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: At the location of 309 E. Douglas Ave in Wichita KS, an employee engaged in sheet metal work on a flat roof was exposed to fall hazards of approximately 8 feet 6 inches. The employee was not adequately trained to use the fall protection provided. The employee demonstrated this by anchoring to an anchor point below him that would not have prevented him from falling.

Douglas Ave in Wichita KS, employees removing construction material off the roof with the use of a crane truck were exposed to struck by hazards. An employee was exposed in that he was standing underneath the suspended load of construction material while standing in a dumpster container.
