29 CFR 1910.132(f)(1)(iii): Employee(s) required to use PPE by this section were not trained to know how to don, doff, adjust, and wear PPE: a. On or about January 14, 2015, Terminal 33, the employer did not train employees to properly don Jackson V90 faceshields in accordance with the manufacturers instructions to ensure they were used within the limits of the ANSI/ISEA Z87.1 certification. Employees who conducted asphalt unloading operations were exposed to eye and face contact with heated asphalt.

29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: a. On or about January 14, 2015, dock, the employer did not ensure face and eye protection was worn by employees during barge to tank asphalt unloading operations which exposes employees to contact with heated asphalt. "b. On or about January 14, 2015, Terminal 33, the employer did not ensure appropriate PPE was used by employee where the Jackson V90 faceshield was available for use. The faceshield was attached to a hardhat, not in accordance with the manufacturer's instructions. Employee was exposed to eye and face contact with heated asphalt during unloading operations."
