29 CFR 1926.451(b)(2):   Scaffold platforms and walkways were not at least 18 inches wide:  Work site located at 235 South 11th Street, Lincoln, NE, 6th Floor:  An employee was observed working from a carpenter bracket scaffold which had a platform less than 18 inches wide.  The platform measured 11 inches and was made up of two 2X6 planks.      Abatement certification is required for this violation.  The abatement certification sheet is enclosed with the citations.

29 CFR 1926.451(f)(7):   Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration.  Such activities were not performed only by experienced and trained employees selected for such work by the competent person.  Work site located at 235 South 11th Street, Lincoln, NE, 6th Floor:  An employee was exposed to fall hazards due to the incorrect erection of a carpenter bracket scaffold.  A person meeting the definition of competent person was not present when the scaffold was erected.      Abatement certification is required for this violation.  The abatement certification sheet is enclosed with the citations.

29 CFR 1926.454(a):   The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.      Work site located at 235 South 11th Street, Lincoln, NE, 6th Floor as well as throughout the company:  Employees working a scaffold had not received training on the relevant portions of Subpart L, Scaffolding.  Without training on the requirements of the standard it is not possible for the employees to ensure that they are working from a scaffold that has been erected correctly.       Abatement certification is required for this violation.  The abatement certification sheet is enclosed with the citations.

29 CFR 1926.1053(a)(8):   A metal spreader or locking device was not provided on each stepladder to hold the front and back sections in an open position when the ladder was being used:   Work site located at 235 South 11th Street, Lincoln, NE, 6th Floor:  A 6 foot-self supporting ladder did not have the metal spreader or locking device engaged.  The ladder was leaned against a carpenter bracket scaffold and was being used for access.      Abatement certification is required for this violation.
