29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.  a. In the turn cell area in the secondary building, the employer did not ensure authorized employees changing a chuck head during set up activities on the MW 200 CNC lathe 69008 were adequately trained to recognize all energy sources, and the methods and means of energy isolation and control.

29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following:  29 CFR 1910.147(c)(4)(ii)(B): Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy.  a. Located in the turn cell area of the secondary building, the MW 200 CNC lathe 69008, lockout/tagout procedures were inadequate in that there were no specific steps and techniques for shutting down, isolating, or blocking all potentially hazardous energy sources.  b. Located in the turn cell area of the secondary building, the MW 200 CNC lathe 69008, lockout/tagout procedures were inadequate in that it did not include the hydraulics as a potential hazardous energy and the need to render them safe.

In the turn cell area in the secondary building, the procedures for locking out the MW 200 CNC lathe 69008, were inadequate in that it did not include specific steps and techniques for the placement and removal of locks on all energy sources.

Located at GKN Sinster in Gallipolis, OH:  a. On the 2014 OSHA 300 log, the employer did not provide all the required information in column F.     1. Case number 14-09 on 10/18/14, did not include what caused the lumbar strain.      2. Case number 14-11 on 11/5/14, did not include which eye was affected.     3. Case number 14-12 on 11/5/14, did not include the actual injury and which finger was affected.     4. Case number 14-13 on 12/4/14, did not include what caused the laceration to the jaw.  b. On the 2015 OSHA 300 log, the employer did not include all the required information in column F.     1. Case number 15-01 on 1/6/15, did not include which fingers were smashed and amputated and did not include the cause of the injury.      2. Case number 15-02 on 1/7/15, did not include which fingers were affected.
