Instance 1: In the door assembly department, a SurgeMaster electrical power strip was used to supply electrical power to a Dayton Shop Floor Fan and other electrical appliances.    Instance 2: In the door assembly department, a Dayton Shop Floor Fan Model # 6K806A was hardwired with a single gang switch to turn on electrical power to the fan.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:  At the Panel Mill, the employer did not ensure that flexible electrical equipment power cords were provided with strain relief where the outer sheathing was pulled back causing strain on the internal wiring, exposing the employees to electrical shock hazards.  Instance #1: A Hampton Bay floor fans flexible cord had strain at the plug;    Instance #2: An Edge Sander flexible cord had strain at the plug; and     Instance #3: A Dayton Shop Floor Fan flexible cord had strain at the fan housing.
