29 CFR 1910.106(e)(6)(i): Adequate precautions were not taken to prevent the ignition of flammable vapors from sources such as open flames; lightning; smoking; cutting and welding; hot surfaces; frictional heat; static, electrical, and mechanical sparks; spontaneous ignition, including heat-producing chemical reactions; and radiant heat:    (a) Employees in the paint area were exposed to fire hazards when rags soaked in flammable paints and stains were stored in a metal container placed directly beside an electrical cord with exposed wiring;    (b) Employees in the paint area were exposed to fire hazards from flammable paints and stains stored on electrical transformers.

Where mechanical handling equipment is used, sufficient safe clearances were not allowed for aisles, at loading docks, through doorways and wherever turns or passage must be made. Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard.
