29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:    Francisco Rivera had an employee exposed to a fall hazard and struck by hazards in the eyes, on a residential construction site in Mahomet, Illinois.  The employer did not have an effective safety and health program with specific work rules, competent person oversight, and employee training to govern employee activities.      Francisco Rivera was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 1926.20(b)(1), which was contained in OSHA inspection number 385363, citation number 1, item number 1A and was affirmed as a final order on June 15, 2012, with respect to a workplace located at 2702 E. Castle Rock, Urbana, IL 61802.

The employer did not have a competent person at the construction site, who was competent in safety rules and requirements, in order to conduct frequent and periodic safety inspections at the construction site.       Francisco Rivera was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.20(b)(2), which was contained in OSHA inspection number 385363, citation number 1, item number 1B and was affirmed as a final order on June 15, 2012, with respect to a workplace located at 2702 E. Castle Rock, Urbana, IL 61802.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:    Francisco Rivera had an employee exposed to struck by hazards in the eyes from the use of a pneumatic gun without eye protection, on a residential construction site in Mahomet, Illinois.  The employer did not provide the employees with the knowledge to recognize construction safety hazards, as well as the procedures to follow in order to minimize or eliminate those safety hazards.     Francisco Rivera was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.21(b)(2), which was contained in OSHA inspection number 385363, citation number 1, item number 2A and was affirmed as a final order on June 15, 2012, with respect to a workplace located at 2702 E.
