Employees using these saws were exposed to severe lacerations and amputations as the lower guards were fixed in the upward position and could not retract immediately after a cut was made.

The extension cord was also plugged into an interior electrical job box with a missing cover. Employees using this equipment were exposed to electrical shock and burn hazards as the employer failed to recognize these hazards.
