29 CFR 1926.304(d): Portable, power-driven circular saw(s) were not equipped with a guard above and below the base plate or shoe:  (a) Cut bench on west side, new residential construction located at 1388 Tanager Bridge Court, Marietta, GA 30062: The employer used shims to keep the lower guards on both Makita Circular Saws from retracting as they were used to cut Hardie-Plank Siding and cornice board materials. Employees using these saws were exposed to severe lacerations and amputations as the lower guards were fixed in the upward position and could not retract immediately after a cut was made.

29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:  (a) Electrical service box on front side, new residential construction located at 1388 Tanager Bridge Court, Marietta, GA 30062: The employer failed to inspect all of the electrical equipment used on site as the extension cord used to power the air compressor and circular saws was missing strain relief and a ground pin. The extension cord was also plugged into an interior electrical job box with a missing cover. Employees using this equipment were exposed to electrical shock and burn hazards as the employer failed to recognize these hazards.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  (a) West side, new residential construction located at 1388 Tanager Bridge Court, Marietta, GA 30062: Employees were exposed to falls of approximately 20-feet, 4-inches while installing Hardie-Plank Siding and cornice boards on a pump jack scaffold platform without a fall protection system in place.
