Worksite located at 8051 N. Denver Ave., Kansas City, Missouri- Employees were exposed to a shock or electrocution hazard in that the employer had failed to examine equipment that had its' outer insulation breached.

Worksite located at 8051 N. Denver Ave., Kansas City, Missouri- Employees were exposed to fall hazards while working on the unprotected edge while installing a guardrailing system and were not connected to any means of anchorage to prevent falling to their deaths.
