29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    On Aug 15, 2016 at the jobsite located at or near 9100 Myrtle Ave in Kansas City, MO employees were exposed to a struck by hazard in that they were using pneumatic nail guns without eye protection.

29 CFR 1926.150(c)(1)(vi): A fire extinguisher, rated not less than 10B, was not provided within 50 feet of wherever more than 5 gallons of flammable or combustible liquids or 5 pounds of flammable gas were being used on the jobsite:  On Aug 15, 2016 at the jobsite located at or near 9100 Myrtle Ave in Kansas City, MO employees were exposed to a fire hazard in that no fire extinguisher was provided when gas was being used on jobsite.
