29 CFR 1910.213(c)(1): Circular handfed ripsaw(s) were not guarded by an automatically adjusting hood which completely enclosed that portion of the saw above the table and above the material being cut: On or about January 23, 2015, in the shop area, the table saw did not have an automatic adjusting hood for the blade. Employee(s) exposed to an amputation hazard from being cut by the saw blade when cutting boards.

29 CFR 1910.305(g)(1)(iv)(B): Flexible cords and/or cables were run through holes in walls, ceilings, or floors: On or about February 12, 2015, at the outside chipping location, a flexible cord was run through the steel wall to the outside. The extension cord was 120 volts and was plugged into a Bosch rotary hammer drill. The outer sheath of the flexible cord was being rubbed against the steel wall. Employee exposed to the potential hazard of an electrical shock.
