At the inspection site, for calendar year 2014, incomplete information for 2 cases on the OSHA 300 Log of Work-Related Injuries and Illnesses materially impaired understandability of the injuries and illnesses in the workplace. a) For both Cases 1 and 2, the specific day of the injury was missing in column D, which states to enter the Date of injury or onset of illness (mo./day). b) For Case 2, the specific injury that occurred was not identified in column F, which states to describe the injury or illness.

29 CFR 1904.29(b)(2): The employer did not fill out or correctly fill out an OSHA Form 301 or equivalent for each recordable injury or illness. At the inspection site, the OSHA 301 Incident Reports or substitute forms were incomplete and not maintained at the detail required by the regulation for each case on the OSHA 300 Logs. a) For calendar year 2014, Case 1 did not have any entries for items 14 through 17, covering what the employee was doing just before the incident occurred, what happened, what was the injury or illness, and what object or substance directly harmed the employee. b) For calendar year 2015, Case 1 did not have any entries for items 6 through 9, 12 and 13, and 14 through 17. Items 6 through 9 address the medical treatment of the employee, items 12 and 13 address the time work began and the time of the event, while 14 through 17 address what the employee was doing just before the incident occurred, what happened, what was the injury or illness, and what object or substance directly harmed the employee. c) For calendar year 2015, Cases 1 and 2 both had dates of injury on item 11 that did not match the OSHA 300 Log date of injury in column D. d) For calendar year 2014, Cases 1 and 2, and calendar year 2015, Case 1, the completion date of the form and the injury date in item 11 were not within 7 calendar days.
