29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level shall be guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder.     On or about January 23, 2015, the employer failed to provide standard railing or other means of equivalent protection for open sided floors and platforms, at the following locations:    a.) the Dryer platform walkway - along the side of the dryer,    b.) the Dryer platform walkway - at the end of the dryer,  c.) the 2nd floor at the magnetic separator - missing midrail,  d.) the elevator platform, and   e.) the Production Screen platform - two sections measuring two and one half feet were not protected.    Due to these conditions, employees have exposure to fall hazards of ten to greater than 20 feet.

The employer's written information for control of hazardous energy does not include specific procedural steps for shutting down, isolating, blocking and securing   machines or equipment to control hazardous energy; and does not include specific steps for lockout device placement, transfer, removal and designate the responsibility for lockout devices; does not include specific requirements for testing and verification of machines or equipment.  Employees perform required duties covered by this part on a daily basis including maintenance, repair and cleaning activities, such as when replacing, adjusting or repairing the hammer mill bars, cradle or hammer.
