29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:     a. On or about July 6, 2017, employees were exposed to a fall of approximately 8.5 feet when working on or around the hammer mill.      b. On or about July 6, 2017, employees were exposed to a fall of approximately 8.5 feet when accessing or working near the pit area.       c. On or about July 6, 2017, employees were exposed to a fall of approximately 6.25 feet when taking a sample from hopper #1.    Steel Valley Firebrick was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.23(c)(1) which was contained in OSHA inspection number 1020332, citation number 1, item number 1, and was affirmed as a final order on June 17, 2015, with respect to a workplace located at 267 Salisbury Rd, Irondale, OH, 43932.

29 CFR 1910.147(c)(6)(ii): The employer did not certify that periodic inspections of the energy control procedures had been performed:   On or about July 6, 2017, the employer had not documented periodic inspections of lockout procedures that had been conducted.
