On or about July 6, 2017, employees were exposed to a fall of approximately 8.5 feet when accessing or working near the pit area.       c. On or about July 6, 2017, employees were exposed to a fall of approximately 6.25 feet when taking a sample from hopper #1.    Steel Valley Firebrick was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.23(c)(1) which was contained in OSHA inspection number 1020332, citation number 1, item number 1, and was affirmed as a final order on June 17, 2015, with respect to a workplace located at 267 Salisbury Rd, Irondale, OH, 43932.
