29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupers for personal protection: a) At the jobsite, on or about 1/14/15: Employees were connecting energized extension cords to aerial lifts (one connection each), where those cords were powered through a connection to an office type power strip, that was sequentially connected to an outlet without GFI protection. Abatement certification must be submitted for this item.

29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables used for temporary wiring were not protected from damage: a) At the jobsite, on or about 1/14/15: Employees were connecting energized extension cords to aerial lifts (one connection to each of 2 lifts), where those cords were passed over and through a snow covered walkway (exposed to foot and vehicle traffic), then passed under an overhead doorway that was closed on the cords. The cords were powered through an office style power strip and to a non GFI protected outlet. Abatement certification must be submitted for this item.
