29 CFR 1910.37(b)(6): Each exit sign must be illuminated to a surface value of at least five foot-candles (54 lux) by a reliable light source and be distinctive in color. Self-luminous or electroluminescent signs that have a minimum luminance surface value of at least .06 footlamberts (0.21 cd/m2) are permitted.  Each exit sign was not illuminated as required:  (a) At the expanded dispatch room west exit door.  (b) At the west radio room.  (c) At the north Initial Attack dispatch room.  (d) At the Helitack bay area.  (e) At the Helitack briefing room.

(a) At the Helitack Bay room, brown storage boxes, gray plastic containers, and blue ice containers/coolers were not stable when stored on top of shelves.  (b) At the janitorial closet, boxes of paper towels and bath tissues were not stable when stored on top of the wooden shelves.

(a) At the Helitack bay two doors leading to the office and the briefing room were not marked or labeled "NOT AN EXIT".  The Bureau of Land Management was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29CFR 1910.37(b)(5) which was contained in OSHA inspection number 315459, citation number 2, item number 2 and was affirmed as a final order on 3/29/2011, with respect to a workplace located at 351 Pacu Lane Bishop, CA 93514.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries.  (a) At the Helitack Bay area, the portable fire extinguisher was blocked by the freezer.  (b) A portable ABC fire extinguisher was free standing next to the gray file cabinet along the walkway of the break room.  The Bureau of Land Management was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.157(c)(1), which was contained in OSHA inspection number 315492298, citation number 1B, item number 2 and was affirmed as a final order on 8/30/2011, with respect to a workplace located at BLM, Caliente, 1400 S. Front Street, Caliente, NV 89008.
