29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor was each employee provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):      a) Greenwood Village residence: On or about January 26, 2015, and at times prior, six workers were exposed to falling 14.7 feet or more while performing roofing activities without a fall protection system. 29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    a) Greenwood Village residence: On or about January 26, 2015, and at times prior, a fall protection roof anchor attached at the ridge was not capable of supporting applied loads as required by a fall protection system. Employees were not trained to properly install temporary roof anchors.    Abatement Note: Abatement certification is required for this item (see enclosed Certification of Corrective Action Worksheet).

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.      a) Greenwood Village residence: On or about January 26, 2015, and at times prior, two workers were not protected from eye injuries when they used manual impact hammers to drive roofing nails into a roof deck.
