29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered:    a) The Toyota forklift, #39, Serial Number 61561, was equipped with ten foot fork extensions, non-factory installed, and the trucks were not marked to identify the attachments or show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered.    b) The Toyota forklift, #38 Toyota, Serial Number 61598, was equipped with ten foot fork extensions, non-factory installed, and the trucks were not marked to identify the attachments or show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered.    c) The Toyota forklift, #40, Serial Number 10409, was equipped with ten foot fork extensions, non-factory installed, and the trucks were not marked to identify the attachments or show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered.

Karavan Trailers, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(a)(4), which was contained in OSHA inspection number 933127, citation number 1, item number 2 and was affirmed as a final order on January 2, 2014, with respect to a workplace located at 100 Karavan Drive, Fox Lake, WI 53933.
