29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, shall be rigged such that an employee can neither free fall more than 6 feet (1.8 m), nor contact any lower level. WORKSITE: The Personal Fall Arrest systems, used by the employees while installing sheathing on wood trusses were not rigged such that the employees could free-fall more than 6 feet or make contact with the floor below.

29 CFR 1926.502(d)(15)(ii): Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, or shall be designed, installed, and used under the supervision of a qualified person: WORKSITE: The wood trusses, used by employees as anchorage points for their respective life lines, were not only incapable of supporting at least 5,000 pounds per person, but also not designed, installed and used under the supervision of a qualified person. Colt Builders Corp. was previously cited for a violation of an equivalent Occupational Safety and Health standard 29 CFR 1926.501(b)(13), which was contained in OSHA Inspection Number315198184, Citation Number 1, Item Number 1, and affirmed as a final order on 6/9/2011 with respect to a workplace located at 105 Cherry Hill Drive, Beverly, Massachusetts.
