29 CFR 1926.451(e)(1):  Portable ladder, hook-on ladder, attachable ladder, stair tower, stairway-type ladder, ramp, runway, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used to access scaffold platforms more than 2 feet above or below a point of access:      a)  Employees were exposed to a fall hazard of approximately 18.3 feet while accessing a ladder jack scaffold.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders shall used at an angle such that the horizontal distance from the top support to the foot of the ladder is approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support):    a)  Extension ladders being used to support a ladder jack scaffold platform were not setup at a 4:1 angle.

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor were the   employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):    a)  Employees were exposed to fall hazards of approximately 12 feet while replacing the gutters to a single family home.    Thunderbolt Roofing & Construction Inc. DBA Thunderbolt Roofing Inc.,  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 282255, citation number 1, item number 1 and was affirmed as a final order on April 17, 2012, with respect to a workplace located at 1137 Franklin Ave., River Forest, IL 60305.
